Social determinants of health (SDOH) are the nonmedical factors that influence health outcomes. They are the conditions in which people are born, grow, work, live, and age. It also includes the wider set of forces and systems, such as developmental agendas, social norms, social policies, racism, climate change, and political systems. Centers for Disease Control and Prevention (CDC) has adopted this SDOH definition from the World Health Organization.
Examples of SDoH we help with are financial difficulties, and lack of things such as food, transportation, clothing, or safe housing.
